Every summer, New Haven Farms (NHF) offers our signature Farm-Based Wellness Program (FBWP), a program designed for overweight adults at risk of developing diabetes. The FBWP meets for two hours each week and is 18 weeks in duration. The program offers sessions in both Spanish and English, in the Fair Haven and Hill neighborhoods of New Haven, respectively. Our program materials and curriculum have been adapted from CDC’s Diabetes Prevention Program (DPP), an evidenced-based lifestyle modification program designed to help individuals make lifestyle changes to both prevent diabetes and prevent or treat the metabolic syndrome. We work with local providers and health clinics that refer their patients to our program.
Eligible program participants:
This program is designed for overweight individuals age 18 and older who have pre–diabetes, diabetes and/or metabolic syndrome, and who are at or below 200% of the poverty line.
To make a referral:
- Fair Haven Community Health Care and Yale New Haven Hospital employees can refer via EPIC, via referral name New Haven Farms.
- If you are a provider that does not use EPIC, you can refer by getting verbal consent from the patient and emailing firstname.lastname@example.org with the patient name, DOB, phone number and risk factors or diagnosis.
- Providers can also tell their patients to text “wellness” or “salud” to 970-00 or call (203) 997-6152 to receive more information!
We are working to improve our referral process, to make it easier for both providers and patients to access our services. Please contact Laura Hansen at email@example.com with any questions or concerns.
